I, ______________________, hereafter referred to as TALENT, have voluntarily participated in the production of a media project and hereby assign the rights to any and all video, audio, photographic, and holographic recordings of my participation made by ______________________ and their associates, hereafter referred to as the FILMMAKERS. 


Talent hereby authorizes the editing, re-recording, alteration, duplication, reproduction, copyright, sale, exhibition, and distribution of said recording(s) for the purpose of production, promotion, and distribution of a media project. Talent waives any right to inspect or approve the finished recordings prior to public release.


Talent understands that the filmmakers can see no risk presently. Talent takes full responsibility for their own involvement in this project and the risks that it may entail, be those legal, physical, or health-related.


Talent has received valuable consideration for participation in this project and expects no additional compensation beyond due credit displayed in the final product.


I hereby certify that I am over eighteen years of age and am competent to contract in my own 
name insofar as the above is concerned. If I am under eighteen years of age and a non-emancipated minor, my parents have given their consent by signing below.
